Ultrasound-Guided Knee Injections

Ultrasound guidance may be used during some knee injection procedures to help visualise the joint or surrounding structures and guide needle placement. It is not required for every injection.

What is an ultrasound-guided knee injection?

Ultrasound imaging uses high-frequency sound waves to create real-time dynamic images of soft tissues, tendons, ligaments, and joint spaces. During an ultrasound-guided injection, the clinician uses these real-time images to visualize the targeted joint anatomy and monitor the needle path.

It is important to understand that ultrasound guidance is a method of delivery rather than an injection product. The injected substance itself may be a corticosteroid, hyaluronic acid, PRP, or Arthrosamid® depending on your clinical indication.

    Preparation and Positioning

    You will be positioned comfortably — typically lying or semi-reclined. The knee area is cleaned with an antiseptic solution. A sterile field is established around the injection site.

    Sterile preparation reduces the risk of infection. A small amount of local anaesthetic may be applied to the skin surface if appropriate.

    Ultrasound Probe Placement

    A water-based transmission gel is applied to the skin. The ultrasound probe (transducer) is positioned over the knee to generate a real-time image of the joint structures and surrounding soft tissues.

    Ultrasound may allow real-time visualisation of the needle and surrounding structures. It does not guarantee a particular clinical outcome.

    Needle Placement — Educational Concept

    Under real-time ultrasound guidance, the needle is introduced through the skin. The clinician monitors the needle position relative to the target joint space or soft tissue structure on the ultrasound screen.

    This is an educational representation. Actual needle depth, angle, and approach are determined by the clinician at the time of the procedure based on your individual anatomy. No diagram can represent guaranteed placement.

    Injection of Therapeutic Substance

    Once the needle position is confirmed as satisfactory, the therapeutic substance is slowly injected into the target space. You may feel a sensation of pressure or mild discomfort during this stage.

    The injected substance depends on your clinical indication — this may be a corticosteroid, hyaluronic acid, PRP, or Arthrosamid®.

    Dressing and Aftercare Instructions

    A small sterile dressing is applied to the injection site. You will receive specific aftercare instructions for your injection type, including rest, activity modification, and warning signs to monitor.

    Most patients can return home shortly after the procedure. Specific activity restrictions depend on the substance used. Contact the clinic if you experience increasing warmth, significant swelling, fever, or severe pain.

How ultrasound guidance works

The procedure is performed in a clean outpatient setting. A water-based transmission gel is applied to the skin over the knee. The clinician moves a hand-held probe (transducer) over the area to send sound waves into the knee, projecting a real-time structural image onto a screen. Under direct visualization, the needle is introduced and guided toward the target tissue recess before the therapeutic fluid is injected. Sterile prep is maintained throughout the process.

When ultrasound guidance may be used

Ultrasound guidance may be considered under several clinical circumstances, such as:

  • Aspiration of fluid from a swollen knee or cyst
  • Targeting specific soft-tissue structures or tendon sheaths
  • Anatomical variants or difficult landmarks (e.g. after prior surgery)
  • When previous landmark-guided attempts have been difficult
  • Where visual verification of joint entry is clinically desired

When it may not be necessary

Many knee joint injections can be performed safely and accurately using anatomical landmarks (palpating physical structures of the knee). Landmark-guided injections are standard clinical practice and do not require additional imaging equipment. The choice depends on clinician judgment, anatomical clarity, and individual patient variables.
