Loose Bodies

Loose bodies are small fragments of cartilage or bone that break off and float freely inside the knee joint, which can cause sudden pain, locking, or swelling.

What is Loose Bodies?

Loose bodies are fragments of cartilage or bone (or both) that have detached from the joint surfaces or other structures and float freely within the joint cavity. They can range in size from a few millimeters to several centimeters. When a loose body becomes caught between the moving surfaces of the knee joint, it can act like a wedge, causing sudden pain, clicking, or mechanical locking where the knee cannot be bent or straightened. Wording is cautious: loose bodies do not always cause symptoms, but if they cause mechanical block or damage to the surrounding cartilage, intervention is usually considered.

  • Osteochondral fracture (injury to cartilage and bone)
  • Osteochondritis dissecans (OCD)
  • Severe osteoarthritis where bone spurs (osteophytes) break off
  • Synovial chondromatosis (rare condition causing multiple cartilage nodules)
